Updated Home Visit Protocol for Newborns
Standard Operating Procedure for Home-Based Newborn Care visits
Updated protocol for conducting Home-Based Newborn Care (HBNC) visits during the first 42 days after birth. Includes revised assessment checklists, danger signs identification, and referral criteria.
1. Visit Schedule
HBNC visits should be conducted on Day 1, Day 3, Day 7, Day 14, Day 21, Day 28, and Day 42 after birth. For low birth weight or premature babies, additional visits on Day 4, Day 5, and Day 6 are required.
2. Assessment at Each Visit
At each visit, assess the newborn for: temperature, feeding pattern, umbilical cord condition, skin color, breathing pattern, activity level, and weight (where possible). Use the standardized assessment form to record findings.
3. Danger Signs
Refer immediately if any of the following danger signs are present: unable to feed, convulsions, fast breathing (more than 60 breaths per minute), severe chest in-drawing, fever or hypothermia, no movement or very lethargic, and skin pustules.
4. Counseling Messages
Key counseling messages for each visit include: exclusive breastfeeding, warmth maintenance (skin-to-skin contact), hygiene practices, immunization schedule, and family planning. Adapt messages based on the newborn's age and condition.
5. Referral Protocol
In case of any danger signs, initiate referral immediately. Use the referral slip format, ensure the referral facility is informed, arrange transport, and follow up within 24 hours of referral.
